To become a new patient at MMR Healthcare, all prospective patients must complete the New Patient Packet. Complete the entire packet ( all the forms ) to the best of your ability. Drop off the completed forms at MMR Healthcare’s front desk during business hours. Be sure to bring an ID and insurance card to be scanned for our records.
Prospective patients may not be scheduled for their first appointment without these forms in our records.
The New Patient Packet allows us to create a complete framework. All these forms make it so that our providers can optimize the healthcare plan from the start of your time with MMR Healthcare.
